Psychological Evaluations for Bariatric Surgery

One of my subspecialties is working with people who are contemplating bariatric surgery for weight loss. I have a relationship with Dr. Michael Snyder of Denver Bariatrics at Rose Hospital in Denver to provide psychological evaluations to determine candidate eligibility.

In this 45- 50 minute appointment, you can expect to discuss your family of origin, your history of weight problems, past successes and obstacles and especially the possible underlying psychological and emotional issues fueling your food addiction. We will discuss HOW you are misusing food, and WHY you are misusing food. I complete the evaluation within one week and send it directly to the office of Denver Bariatrics.

I will accept Anthem BCBS and Aetna insurance for the evaluations, however it is the patient’s responsibility to obtain pre-authorization approval and to know before the evaluation takes place, of any deductible that must be met and/or the amount of the copay. If you plan to pay cash, my fee is $200 for cash and checks, $210 for credit cards and flexible spending cards. If you wish to return to me for follow-up counseling, I will accept some insurances (please see Policy page) and you will only be responsible for your copay.
